Our mission of being a Patient-Centered Medical Home is of combining the fundamentals of access to care, meaningful use of electronic health records, and care management of chronic diseases and preventive screening services.  Our certification through NCQA has allowed us to deliver a new and improved means of delivering health care to you and your family here at Finger Lakes Family Care.  Below are highlighted areas of what being a Medical Home means to you and what we expect patients to do in order to reap all of the extra benefits of our national accreditation:
Open Access
Being able to see patients the day they call for urgent matters is something we value and have changed the way our schedules are set each day in order to accommodate same-day visits.  We therefore will expect patients to schedule visits with us during our office hours before considering urgent care or emergency rooms as we know our patients’ medical histories better and are thus better able to treat them without ordering unnecessary or redundant tests.
Care Management
Care Management will include proactively monitoring and managing chronic disease such as diabetes, hypertension, and asthma along with Dr. Smith and the providers of Finger Lakes Family Care.  Please help us welcome her to our practice when you see her.
The Medent Patient Portal
The Patient Portal will allow secure, online access to the following:
  • Personal Health Records and facesheet containing medical diagnoses, medication list, immunization records, and more
  • Prescription refills with one click of the mouse
  • Appointment requests
  • Secure messaging to our clerical, nursing, and medical staff
