Finger Lakes Family Care is proud to continue being a charter member in the Rochester Medical Home Initiative's pilot project. We have been awarded Level 3 Recognition by the National Committee for Quality Assurance (NCQA) as a Physician Practice Connections® - Patient Centered Medical Home™, emphasizing the use of systematic, patient-centered, coordinated care that supports access, communication, and patient involvement.
Office Hours
Our office is open from 8:00 AM to 5:00 PM, Monday through Friday. We close for lunch between 12:15 PM and 1:00 PM each day.
Patient Portal
The Finger Lakes Family Care Patient Portal allows for secure, online access to your medical records. Personal Health Records are kept updated in real-time for access while at home, at work, or on vacation. Your records are no more than a mouse-click away!
