Our mission as a Patient-Centered Medical Home consists of combining the fundamentals of access to care, meaningful use of electronic health records, and care management of chronic diseases and preventive screening services. Our Level 3 Certification through NCQA has allowed us to deliver a new and improved means of delivering health care to you and your family here at Finger Lakes Family Care. Patient Portal
The Finger Lakes Family Care Patient Portal allows for secure, online access to your medical records. Prescription refills and appointments can be requested at the touch of a button. Personal Health Records are kept updated in real-time for access while at home, at work, or on vacation. Your records are no more than a mouse-click away! The Portal also allows for online chat as well as virtual care and video conferencing.

Finger Lakes Family Care is proud to have been chosen as one of seven practices in the Rochester Medical Home Initiative's pilot project. We have been awarded Level 3 Recognition by the National Committee for Quality Assurance (NCQA) as a Physician Practice Connections® - Patient Centered Medical Home™. The PPC-PCMH standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication, and patient involvement.
