Finger Lakes Family Care is a "Patient Centered Medical Home", offerring same day appointments for sick and acute visits, chronic disease management supervised bu our in-house care manager, and offerring secure email communications via the UpDox Network with our patients to ensure timely and efficient care.  We are proud to have been chosen by Excellus and MVP as one of a few practices in the Western New York area, and first in the Finger Lakes to be included in their Medical Home Pilot Project.  We will be working towards re-engineering our practice to meet NCQA Standards towards becoming a certified medical home for the delivery of primary care.
